Office Hours, Appointments and Cancellation Policy
If you can’t keep your appointment, please call the office 24 hours in advance so we can schedule another patient. If you no show for any appointment without
calling to cancel at least 24 hours prior, you will be charged a $20.00 “No Show” fee.
If you no show for a scheduled surgical procedure without calling 24 hours
prior to cancel, you will be charged a $75.00 “No Show Surgical” fee.
Despite schedules, emergencies can cause delays. If your appointment is affected, we’ll try to keep you informed. We know that your time is valuable too.
If you no show for your first appointment as a New Patient without calling 24 hours prior to cancel, you will not be allowed to schedule future appointments.
For your first visit, plan to arrive at least 15 minutes early. Our office staff will ask you to fill out an information form about your medical history. We need information about your insurance coverage also. Please bring your insurance card. We ask our patients to fill out update forms once every year. However, should any of this information change between visits,
please let us know so we can update your records.
Your medical records are kept confidential. When you sign our patient information form, you are authorizing
us to release medical information to your primary care or referring physician, to consultants if needed and as
necessary to process insurance claims and prescriptions.
Check your medication supply before each appointment so you can arrange for refills while at the office. If you run out of a routine medication between visits, please contact your pharmacy, they will call us for a refill. Please arrange for refills at least 48 hours before they are needed. Prescriptions can not be refilled after hours or on weekends.
Payment is due at time of service
We offer a 20% discount for cash payments and a 15% discount for credit/debit card payments when you pay in full for medical procedures at the time of service. These discounts are not available on skincare products.
We collect deductibles and copayments/coinsurance from the patient at the time of service. If you are unsure how much your deductible/copayment/coinsurance will be under your plan, please contact your health plan.
We cannot waive these fees and there are penalties for providers who do not make an effort to collect these payments from the patient.
Insured and uninsured patients alike have a number of financing options available to them to ensure that you receive the skin care you need that fits your budget.

We accept the following insurance plans
